We accept Medicare, Blue cross Blue shield, worker's compensation, Auto insurance and PPO Plans. We will be happy to verify insurance coverage for your treatment at Lakeside Physical Therapy prior to you first appointment. We will inform you if your particular insurance plan requires a pre-authorization for particular services.
As a courtesy to you, we will submit all charges to your insurance company. However, many insurance plans require you to satisfy a deductible, co-insurance and/or co-pay as part of coverage.
